VNA has the only pediatric multi-disciplinary home health team in this region. Services are available to children until their 21st birthday, according to a plan of care developed with the physician and family. Services can range from intensive high-tech treatment therapies and complex post-surgical care to assistance with daily living activities — the VNA pediatric home care program can provide the appropriate caring professional for each situation. VNA provides specialized care for children with life-threatening conditions from congenital problems, post-operative acute care, and chronic illness such as diabetes, cystic fibrosis and cancer. VNA pediatric nurses make more than 3,000 pediatric visits each year, maintaining regular contact with referring physicians and their hospitals locally as well as the UC San Francisco Medical Center and the Lucile Salter Packard Children's Hospital at Stanford Medical Center. Home nursing allows the child to receive specialized health care services in familiar, nurturing surroundings with the support and love of their families. VNA's Pediatrics and Obstetrics Programs offer:
Maternal Child Health Program: VNA's Maternal Child Health Program makes approximately 1,300 in-home visits by registered nurses each year to new mothers and their newborn child following discharge from area hospitals. With hospital stays for childbirth diminishing, the focus of the visits is to provide physical assessment of mom and baby, post-partum care and follow-up, and parent education. The aim of the program is to assist in decreasing the number of children who experience preventable childhood diseases, chronic health conditions and injuries. After completing a comprehensive assessment of the mohter and the newborn, the nurse offers breastfeeding instruction and arranges for the mother and the baby's follow-up physician appointments and immunizations. The nurse also completes, if appropriate, a high-risk assessment and refers those infants to the Public Health Department or Child Protective Services, if necessary. There is a two-month follow-up survey phone call to assess the mom's and the baby's progress.
