Hospice volunteers are trained to provide support and
companionship to hospice clients and their families. The
VNA Hospice program welcomes as volunteers all persons
aged 18 and older who have successfully completed an
interview process, an orientation session and 38 hours
of training.

the online Hospice Volunteer Application form.
This application is vital in our understanding you as an
individual, so it is worth your while to take some time
and compose your thoughts before answering the essay
questions honestly and thoughtfully.
The application can be filled out online and submitted
electronically by using the Submit button at the bottom,
or it can be printed and mailed to the address on the
form. Please note that we cannot accept applications via
fax.

3. Click the following link to the Hospice Volunteer
Reference form and print three of
the forms.
Complete the top part of each Hospice Volunteer
Reference Request form. The bottom half of the forms
need to be completed by the individuals/organizations
you’ve selected as references. Please instruct those
individuals/organizations to mail the forms directly
back to the Visiting Nurse Association. Volunteer
Reference Forms submitted directly by the prospective
volunteer WILL NOT be accepted.

After we have received your application and reference
forms, they will be reviewed. You will receive a
confirmation letter further explaining the process.
