MONTEREY
PCP Supervisor – Monterey FT Oversees the management of the Patient Care Planning department and personnel. Represents the PCP department in meetings with other departments. Is responsible for the intake and referral of patients into the organization. Accepts referrals from patients, families, hospitals, physicians and other referral sources. Provides information about services and facilities. Develops and maintains rapport with members of the medical community to identify ways of best caring for patients. Knowledge of Medicare/ MediCal regulations and guidelines Knowledge of prior authorization for insurance coverage. Strong attention to detail. Recommends improvements in the process to director by recommending changes in policies and procedures. Coordinate all activities of the admission process from point of referral to completion of paper work and transfer to the clinical branch office. Education/Experience/Skills/Qualifications Graduation from a school of nursing accredited by the National League for Nursing. BSN preferred. Possession of a current and valid license to practice as a Registered Nurse or Public Health Nurse in the State of California. Two years of home health or hospice experience within the last three years. Knowledge of reimbursement criteria for home health care. Knowledge of community resources. At least one year of supervisor experience within last three years preferably in a home health care setting.

Assists the patient in attaining maximum potential through application of physical therapy principles. Consults with patient's family and caregivers and other health team members. Evaluates home environment & makes recommendations. CA PT license and CPR required. Minimum of one year of experience, preferably in an acute rehabilitation setting. Home health training is available
Physical Therapist Assistant - FT(1)
Perform physical therapy procedures to patients according to the treatment plan and as directed by the Physical Therapist. Record and report activities. Provides input/feedback to other health team members. Occupational Therapist –FT
Assists the patient in attaining maximum potential through application of occupational therapy principles. Consults with patient's family and caregivers and other health team members. Evaluates home environment & makes recommendations. CA OT license and CPR required. Minimum of one year of experience, preferably in an acute rehabilitation setting. Home health training is available
Occupational Therapist Assistant – FT (1)
Provides occupational therapy to patients under the prescribed plan of treatment and as directed by the OTR and approved by a physician. Assists the patient in attaining maximum potential through application of occupational therapy principles. Provides consultation to other health team members. Current CA OT Assistant license and registration by the American Occupational Therapy Association. Minimum of one year of recent acute, SNF or home health agency experience.

HOSPICE
Hospice Clinical Supervisor Job Summary Clinical management of the Hospice program under the supervision of the Director of Patient Services. Serves as back-up to the director in the absence of the director when requested. Manages clinical aspects and Quality Assurance Performance Improvement program for hospice. Supervises scheduling and oversees staff to ensure optimal utilization of social work, spiritual care and hospice aide in meeting productivity requirements. Facilitates clinical management of patients and admissions to hospice. Attends IDT, interdisciplinary meetings with the staff and hospice Medical Director. Participates in patient care assessments, planning, interventions and evaluations. Understands the coordination of care in the homes, skilled nursing facilities, residential care facilities and hospitals. Knowledge of hospice conditions of participation, the compliance rules and regulations, and agency policies and procedures. Responsible for chart audit, compliance and medical records. Promotes favorable public relations and serves on various committees. NOTE: An employee filling this position on a full-time basis is considered exempt from the overtime provisions of the Fair Labor Standards Act. Education/Experience/Skills/Qualifications Possession of a license to practice as a Registered Nurse in the State of California. Bachelor of Nursing degree from a program accredited by the National League for Nursing preferred. Possession of CPR certification. At least 2 years of recent experience in hospice care. At least 1 year of supervisory experience or equivalent training in a hospice agency. Knowledge of Medicare, Medi?Cal, and private insurance coverage.
Primary Case Manager- Hospice - F/T Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families. Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver. Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers. Case manages patient care within the guidelines of the various payor sources. Prepares patient/family for discharge when appropriate. Commitment to the interdisciplinary team approach to care. Strong communication skills. Teaching and leadership abilities. Education/Experience/Skills/Qualifications Registered Nurse: Possession of a license to practice as a Registered Nurse in the State of California. Two years of experience as a professional nurse in a medical?surgical facility and one to two years of recent home care experience. Hospice experience preferred. Preferred knowledgeable about all agency services. Possession of current CPR certification.

Volunteer Coordinator – Hospice (1 PT) Recruits and selects volunteers by utilizing community resources. Trains and evaluates volunteers. Maintains records according to established schedules. Responsible for developing and maintaining gross profit reports for the hospice program. Responsible for accuracy of billing and reimbursement for Hospice services. Responsible for developing and maintaining gross and net profit reports, allowing for individual client tracking. Responsible for and maintains CQI plan for the hospice program.
