Physical Therapist –FT, PT .8 or PD Assists the patient in attaining maximum potential through application of physical therapy principles. Consults with patient's family and caregivers and other health team members. Evaluates home environment & makes recommendations. CA PT license and CPR required. Minimum of one year of experience, preferably in an acute rehabilitation setting. Home health training is available Occupational Therapist –FT. PT.8 Assists the patient in attaining maximum potential through application of occupational therapy principles. Consults with patient's family and caregivers and other health team members. Evaluates home environment & makes recommendations. CA OT license and CPR required. Minimum of one year of experience, preferably in an acute rehabilitation setting. Home health training is available
MONTEREY
Primary Care Nurse – (1) FT with rotating weekends – Monterey Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families. Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver. Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers. Case manages patient care within the guidelines of the various payor sources. Prepares patient/family for discharge when appropriate, referring to community resources. Registered Nurse: Possession of a license to practice as a Registered Nurse in the State of California. Two years of experience as a professional nurse in a medical?surgical facility within the last three years and one to two years of recent home care experience. Preferred knowledgeable about all agency services. Possession of current CPR certification. BSN, PHN preferred.
Medical Social Worker – Monterey - FT Assesses the social and emotional factors related to patient’s illness, need for care, capacity and potential to cope with problems of daily living. Works with team members in development of treatment plans and participates in conferences. Education/Experience/Skills/Qualifications
