New Positions:

 

 

 

QA RN – Monterey FT.

Reviews client charts to assure proper documentation to maximize reimbursement, quality of  care provided, visit utilization, appropriate documentation with physicians, adherence to care plan, and evidence of communication between disciplines.  Reviews ICD-9 coding for accuracy.  Responsible for the oversight and analysis of all OASIS data.  Responsible for the evaluation of OASIS improvement initiatives.  Coordinates OASIS requirements and procedures in two branch offices.  Participates in QI AND PI audits and activities.  Duties will include RAC Audits, HHCAHPS, ADRs, Subpoenas, chart review, utilization and compliance.  Registered Nurse with a current California license.  Minimum 2 years experience in home health care setting.  Experience as a QA RN and BSN preferred.  OASIS certification required, or ability to obtain within six months of hire.  Coding certification or ability to obtain within six months of hire.  Excellent verbal and written communication skills.  Strong organizational skills.  Knowledge of various payor guidelines including but not limited to Medicare, Medi-Cal, managed care, etc is required.  Must be detail oriented and meet productivity standards and agency goals.  Important to function well independently, yet also work with a team.  Occasional overtime and possible occasional weekends.

Home health training is available

Occupational Therapist –FT. PT.8

Assists the patient in attaining maximum potential through application of occupational therapy principles.  Consults with patient's family and caregivers and other health team members.  Evaluates home environment & makes recommendations.  CA OT license and CPR required.  Minimum of one year of experience, preferably in an acute rehabilitation setting. Home health training is available

 

  MONTEREY 

 

 Primary Care Nurse – (1) FT with rotating weekends – Monterey

Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families.  Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver.  Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers.  Case manages patient care within the guidelines of the various payor sources.  Prepares patient/family for discharge when appropriate, referring to community resources.

Registered Nurse: Possession of a license to practice as a Registered Nurse in the State of California.  Two years of experience as a professional nurse in a medical?surgical facility within the last three years and one to two years of recent home care experience.  Preferred knowledgeable about all agency services.  Possession of current CPR certification.  BSN, PHN preferred.

 

 

Medical Social Worker – Monterey - FT

Assesses the social and emotional factors related to patient’s illness, need for care, capacity and potential to cope with problems of daily living.  Works with team members in development of treatment plans and participates in conferences.

Education/Experience/Skills/Qualifications
Bachelor’s of Science in Social Work with 1 year of health care experience or Master's degree from a school of social work accredited by the Council Social Work Education.  One year of social work experience in a health care setting, preferably with a home health agency.   Possession of a current CPR certification.

 

SALINAS
