Hospice Services
Selecting VNA Hospice means you will be involved with our compassionate and knowledgeable staff, and participating in a health care model long considered to be the standard for excellence in care at the end of life. Our goal is to offer a multi-discipline team approach to provide our patients with dignity, support, and illness management in the home setting. Support is also provided to the patient's loved ones, encouraging their participation in the hospice model of care. Our hospice is Medicare Certified, with services being billed to the Medicare Hospice Benefit, Medi-Cal, or Private Insurances. The Hospice TeamThe VNA Hospice team attends to the physical, emotional, and spiritual needs of our patients by working with the patient/family to establish an individualized plan of care. Our hospice team includes: the Primary Physician, Hospice Medical Director, Hospice Nurses, Certified Hospice Aides, Social Workers, Chaplains and Spiritual Counselors, Specially Trained Volunteers, and Bereavement Counselors.
